The sensible move is to weigh symptoms plus the ferritin trend, not defend one red line — while resisting the urge to blame all fatigue on iron and over-supplement. Ask the cause first, then decide whether to treat.
② If ApoB is more accurate, why do most panels still give only LDL-C?
Inertia and cost. LDL-C is cheap, has vast historical data, anchors the guidelines, and every clinician knows it; ApoB needs an extra assay and the "particle count" idea is less familiar. It's a classic lag between evidence and practice — what you can do is request the add-on and decide with the more mechanism-aligned metric, rather than wait for the default to update.
③ What does aspirin's fall from "one for everyone" to "no longer default" teach us?
A "sound mechanism" isn't the same as "net benefit."
